Objective Gather information from physicians about factors contributing to unnecessary noninvasive imaging and impact of possible solutions. Methods Qualitative study of 14 physicians using a phenomenological approach and the Theoretical Domains Framework. Results Most participants self-reported that >10% of the imaging tests they order are unnecessary. External sources of pressure included: peer-review, patient demands, nursing expectations, specialist requests, as well as prior experience with patient advocates, and the compensation and pension system. Internal sources of pressure included reliance on anecdote, self-doubt (...) about diagnoses, and fear of missing a diagnosis and of professional liability. Participants expressed both optimism and concern about potential solutions, such as adopting decision support software. Conclusion Physicians are under pressure from multiple sources to order unnecessary imaging. Peer review, nursing expectations, and perceptions about Veteran compensation and pension are newly reported contributing factors. (shrink)

Mexico's pandemic management and the absence of measures have been harshly criticized as being disproportionate. This paper examines whether the proportionality principle was properly applied to Mexico's COVID-19 response and outlines three reasons against such an endeavor, namely (i) the content of “proportionate measures” remained insufficiently well defined, (ii) there were yet fundamental rights conflicts to resolve, and (iii) the situation was moreover characterized by epistemic uncertainty.

In this article we discuss the contextual character of quantum mechanics in the framework of modal interpretations. We investigate its historical origin and relate contemporary modal interpretations to those proposed by M. Born and W. Heisenberg. We present then a general characterization of what we consider to be a modal interpretation. Following previous papers in which we have introduced modalities in the Kochen-Specker theorem, we investigate the consequences of these theorems in relation to the modal interpretations of quantum mechanics.

Proponemos que la narrativa monumental ha sido tensionada por el despliegue de una producción estética que ha revelado sus violencias y exclusiones históricas. El centro de Santiago fue el escenario donde se desplegaron diversos repertorios que hablan de las múltiples temporalidades que habitamos, entre ellas la temporalidad colonial. En (...) este sentido, creemos que la Revuelta tuvo un carácter chi’xi, siguiendo las ideas de Silvia Rivera Cusicanqui, es decir, operaron repertorios y estéticas yuxtapuestas desde donde se impugnaron las políticas de identidad del estado chileno. Asimismo, como parte de este proceso, se desplegaron nuevos usos, significados y nombres en el espacio urbano, a modo de un quehacer insurgente cuya densidad retorica todavía nos desborda, sigue abierto y en disputa. La violencia en México, específicamente en algunos estados de la República mexicana en donde la población se enfrenta a situaciones de peligro, tiene como una de tantas consecuencias la ausencia de personal médico que se desempeñe como profesional y proporcione servicios de salud en estas zonas rurales. En este sentido, es de considerar que la asignación de recursos humanos en salud es fundamental para el funcionamiento de un sistema eficiente y observar las causas por las que médicos que desde el (...) servicio social hasta el desempeño de su especialidad no consideran trabajar en lugares en donde se sienten vulnerables. En este proceso, es importante visibilizar los cuestionamientos éticos que pueden surgir por lo que este trabajo pretende dar importancia al tema de la exposición de circunstancias, hechos y cifras dentro de esta problemática y plantear las cuestiones normativas pertinentes dentro de este contexto. In Aristotelian logic, categorical propositions are divided in Universal Affirmative, Universal Negative, Particular Affirmative and Particular Negative. Possible relations between two of the mentioned type of propositions are encoded in the square of opposition. The square expresses the essential properties of monadic first order quantification which, in an algebraic approach, may be represented taking into account monadic Boolean algebras. More precisely, quantifiers are considered as modal operators acting on a Boolean algebra and the square of opposition is represented by relations (...) between certain terms of the language in which the algebraic structure is formulated. This representation is sometimes called the modal square of opposition. Several generalizations of the monadic first order logic can be obtained by changing the underlying Boolean structure by another one giving rise to new possible interpretations of the square. In this paper we solve the problem how to axiomatize a system of quantum computational gates known as the Poincaré irreversible quantum computational system. A Hilbert-style calculus is introduced obtaining a strong completeness theorem.

In the framework of algebras with infinitary operations, the equational theory of $$\bigvee _{\kappa }$$ ⋁ κ -complete Heyting algebras or Heyting $$\kappa $$ κ -frames is studied. A Hilbert style calculus algebraizable in this class is formulated. Based on the infinitary structure of Heyting $$\kappa $$ κ -frames, an equational type completeness theorem related to the $$\langle \bigvee, \wedge, \rightarrow, 0 \rangle $$ ⟨ ⋁, ∧, →, 0 ⟩ -structure of frames is also obtained.

An algebraic setting for the validity of Pavelka style completeness for some natural expansions of Łukasiewicz logic by new connectives and rational constants is given. This algebraic approach is based on the fact that the standard MV-algebra on the real segment [0, 1] is an injective MV-algebra. This essay is followed by the first complete English translation and publication of Donoso’s letter of October 24, 1851, which contains Donoso’s only reference to the “discussing class,” a political entity later popularized by Schmitt in his 1922 work Political Theology. In this paper we attempt to physically interpret the Modal Kochen–Specker theorem. In order to do so, we analyze the features of the possible properties of quantum systems arising from the elements in an orthomodular lattice and distinguish the use of “possibility” in the classical and quantum formalisms. Taking into account the modal and many worlds non-collapse interpretation of the projection postulate, we discuss how the MKS theorem rules the constraints to actualization, and thus, the relation between actual and possible (...) realms. We present a generalization of the problem of pattern recognition to arbitrary probabilistic models. This version deals with the problem of recognizing an individual pattern among a family of different species or classes of objects which obey probabilistic laws which do not comply with Kolmogorov’s axioms. We show that such a scenario accommodates many important examples, and in particular, we provide a rigorous definition of the classical and the quantum pattern recognition problems, respectively. Our framework allows for the introduction of (...) non-trivial correlations between the different species involved, opening the door to a new way of harnessing these physical resources for solving pattern recognition problems. Finally, we present some examples and discuss the computational complexity of the quantum pattern recognition problem, showing that the most important quantum computation algorithms can be described as non-Kolmogorovian pattern recognition problems. The Cuban government has undertaken the task of transforming insect pest and weed management from conventional to organic and more sustainable approaches on a nationwide basis. This paper addresses past programs and current major areas of research and implementation as well as provides examples of programs in insect and weed management. Topics covered include the newly constructed network of Centers for the Reproduction of Entomophages and Entomopathogens (CREEs), which provide the infrastructure for the implementation of biological control on state, cooperative, (...) and private farm operations. CREE programs include the mass rearing and release of Trichogramma spp. to manage insect pests of cassava, tobacco, sugarcane, and improved pasture. The entomopathogens, Bacillus thuringiensis, Beauveria bassiana, Metarhizium anisopliae, and Verticillium lecanii, are being mass produced in the CREEs and applied to control a number of key insect pests. A traditional method employed by peasant farmers of using the predatory ant Pheidole megacephala (Fabricius) in the biological control of sweetpotato and banana insect pests has been further developed and implemented. Efforts are underway to rear nematode species that attack insect pests and identify plant extracts with insecticidal qualities as potential management tools. Weed management has moved away from dependence on herbicides and toward alternative techniques that include weed suppression based on rotation schemes, limited use of selective herbicides, and new tillage equipment and practices. In this paper we attempt to analyze the concept of quantum probability within quantum computation and quantum computational logic. While the subjectivist interpretation of quantum probability explains it as a reliable predictive tool for an agent in order to compute measurement outcomes, the objectivist interpretation understands quantum probability as providing reliable information of a real state of affairs. After discussing these different viewpoints we propose a particular objectivist interpretation grounded on the idea that the Born rule provides information about an (...) intensive realm of reality. We then turn our attention to the way in which the subjectivist interpretation of probability is presently applied within both quantum computation and quantum computational logic. Taking as a standpoint our proposed intensive account of quantum probability we discuss the possibilities and advantages it might open for the modeling and development of both quantum computation and quantum computational logic. In this paper we enrich the orthomodular structure by adding a modal operator, following a physical motivation. A logical system is developed, obtaining algebraic completeness and completeness with respect to a Kripkestyle semantic founded on Baer*-semigroups as in [22].

In this paper we analyze and discuss the historical and philosophical development of the notion of logical possibility focusing on its specific meaning in classical and quantum mechanics. Taking into account the logical structure of quantum theory we continue our discussion regarding the Aristotelian Square of Opposition in orthomodular structures enriched with a monadic quantifier. Finally, we provide an interpretation of the Orthomodular Square of Opposition exposing the fact that classical possibility and quantum possibility behave formally in radically different manners.

In this note we present a completion for the variety of bounded distributive quasi lattices, and, inspired by a well-known idea of L.L. Maksimova [14], we apply this result in proving the amalgamation property for such a class of algebras.

Some theories of goodness are descriptively rich: they have much to say about what makes things good. Neo-Aristotelian accounts, for instance, detail the various features that make a human being, a dog, a bee good relative to facts about those forms of life. Famously, such theories of relative goodness tend to be comparatively poor: they have little or nothing to say about what makes one kind of being better than another kind. Other theories of goodness—those that take there to be (...) absolute goodness—are comparatively rich: they offer grounds for judging some types of things better than others because they have more absolute goodness. Moorean accounts, for example, can tell us that humans and human experiences are superior to bees and blades of grass. But such theories tend to be descriptively poor: they struggle to tell us in virtue of what this is so. In this essay we motivate and flesh out a view that splits the difference between accounts of goodness as relative and accounts of goodness as absolute. Such a view holds promise only if the mechanics of this kind of metaphysics of goodness can be worked out. Here we present a view on which the paradigm for absolute goodness is God and the paradigm for each kind of relative goodness is a divine idea. (shrink)

Throughout a distinguished career, Bob Hale’s work has addressed this question on a number of fronts, through the development of a Fregean approach to ontology, an essentialist theory of modality, and in his work on neo-logicism in the philosophy of mathematics. This collection of new essays engages with these themes (...) in Hale’s work in order to make further progress in our understanding of ontology, modality, and the relations between them. Some essays directly address questions in modal metaphysics, drawing on ontological concerns. Others raise questions in modal epistemology and its links to matters of ontology, such as the challenge to give an epistemology of essence. There are also several essays engaging with questions of what might be called ‘modal ontology’: the study of whether and what things exist necessarily or contingently. Such issues can be raised and addressed directly, but they also have an important bearing on the kinds of semantic commitments engendered in logic and mathematics, e.g., to the existence of sets, or numbers, or properties, and so on. It is thus explored in some chapters to what extent one’s ontology—and indeed, one’s ontology of necessary beings—interacts with other plausible assumptions and commitments. (shrink)

Adoption of integrated pest management(IPM) practices in the Guatemalan highlands has beenlimited by the failure of researchers andextensionists to promote genuine farmer participationin their efforts. Some attempts have been made toredress this failure in the diffusion-adoptionprocess, but farmers are still largely excluded fromthe research process. Understanding farmers'agricultural knowledge must be an early step toward amore participatory research process. With this inmind, we conducted a semi-structured survey of 75Cakchiquel Maya farmers in Patzún, Guatemala, tobegin documenting their pest control practices. Theirresponses revealed (...) that their understanding ofbiological and curative pest control is limited.However, their broad knowledge of cultural preventivepest control practices could explain why they hadfaced few pest problems in their traditionalmilpa (intercrop of corn, beans, and other edibleplants). Since 1900, debates about the justification of punishment have also been debates about the cause of crime. In the early twentieth century, the rehabilitative ideal of punishment viewed mental illness and dysfunction in individuals as the cause of crime. Starting in the 1970s, retributivism identified the immorality of human agents as the source of crime, which dovetailed well with the “tough-on-crime” political milieu of the 1980s and 1990s that produced mass incarceration. After surveying these historical trends, Wirts argues for a (...) social critical view that crime is best understood as a product of an unjust society, not faulty human beings. This view should take the lessons learned by the critiques of rehabilitation, however, and resist the tendency to reduce human beings to recipients of social services. (shrink)

The topic of professionalism has dominated the content of major academic medicine publications during the past decade and continues to do so. The message of this current wave of professionalism is that medical educators need to be more attentive to the moral sensibilities of trainees, to their interpersonal and affective dimensions, and to their social conscience, all to the end of skilled, humanistic physicians. Urgent calls to address professionalism from such groups as the Association of American Medical Colleges, the American (...) Board of Internal Medicine, and the Accreditation Council for Graduate Medical Education, among others. In fact, at the 2004 annual meeting of the AAMC six separate presentations addressed professionalism with such titles as "Evaluating Humanism and Professionalism," Professionalism: Expectation, Education, Evaluation," or "Toward Assessing Professional Behaviors of Medical Students through Peer Observations". Professionalism, then, has become part of the current academic medicine parlance, used by administrators, clinical faculty, residency programs, and professional organizations with an expectation of shared meanings and goals. All of these stakeholders focus on what has become a consistent list of attributes deemed to be the essence of professionalism, which usually include variations on altruism, duty, excellence, honor and integrity, accountability, and respect. In fact, most of the scholarly work to date has been listing, describing, decrying, and measuring/evaluating it. In this collection of essays, we don’t argue with these attributes. Instead, we ask questions of the discourse from which they arise, how the specialized language of academic medicine disciplines has defined, organized, contained, and made seemingly immutable a group of attitudes, values, and behaviors subsumed under the label "professional" or "professionalism." This collection aims to be a critical text, one that questions the profession’s beliefs about the nature of its work and how such beliefs are enacted in medical education, particularly as they fuel the professionalism discourse. In addition, we will scrutinize how the discourse is enacted in both the formal and hidden curriculum, and in the larger medical environment. (shrink)
